The survey is part of the research conducted at the Warsaw School of Economics in Poland and its only purpose is to collect your opinion on the Usage-Based Insurance (UBI). The questionnaire is anonymous - the information provided herein will not allow for the identification of your identity.

The questionnaire, apart from the 'Additional Information' Section, consists of seven questions and its completion should not take more than 5 minutes.
