UNO Mentor Profile: Add, update, confirm, remove

This form is to be used by UNO faculty and staff members who wish to:

1. Participate in the Tolmas Scholars programs (PURSUE/CoSURP) from January 2, 2023 - December 31, 2023.

2. Participate in Mentor Match program, which connects students with mentors for projects throughout the year. This program has no Office of Research funding associated with it.

3. Participate in the McNair program.

4. Remove their names from the mentor listing for either program.
