This form is so that I may gain your student's information for communication purposes and information to better help me teach your child. This information is only for Mr.Carrillo's records.
There is a district wide participation fee of:
$15 for elementary students (4th -5th grade)
$25 dollar participation fee for middle school students (6th-8th grades).
The fee is due to Mr. Carrillo by the instrument Pickup Night on September 21st (at King Arts 3:30pm-6:30pm) preferably in cash.
Instrument pick-up night will be on:
Wednesday, September, 21st at King Arts (at King Arts, Lake street side) from 3:30-6:30pm.
Seman's Violin company and myself will be doing a drive by pick up style for all instruments.
A note:
*Scholarship instruments will be handed out first come first serve basis, and with a preference to students who have been playing longer in the program. Students in need of a scholarship instrument need to simply fill out this google form, pay the activity fee, and Mr. Carrillo will contact you directly for more information.
