Questionnaire 'Business of School Lunches' for Primary Schools.

To Parents/Guardians/Caregivers.

Kia Ora,

I am Julie Bantegny Weimann, a Southern Institute of Technology | Te Pūkenga student, studying for a Master of Applied Management.

I am conducting a research project examining parents’ attitudes towards school lunches in Queenstown primary and secondary schools. I am looking for parents over 18 years old with at least one child enrolled in a primary school in Queenstown to complete this questionnaire. It will take only ten to fifteen minutes, and your answers will remain confidential and anonymous.

The results will be written and presented in a research report that is assessed as a part of my programme of study. The finished dissertation may also be available in the Southern Institute of Technology’s Research Bank.

You do not need to participate in this study, but if you do, then I thank you. You do not need to answer any questions you do not want to answer. If you complete the questionnaire, you consent to have your responses included within my research data.

This project is being carried out under ethical approval from the Southern Institute of Technology’s Human Research Ethics Committee. The data gathered will be stored on a password-protected computer for five years, after which it will be destroyed.
