This form does not save and takes about half an hour to fill out by both student & parent. Please start the form when both student and parent have the time and have fully read the Adult Mentor Roles at www.ftcroboticsedina.com/adult-roles. The purpose of this form is for students in 7th-12th grades, in any Edina Public School, to express COMMITMENT to the FIRST Tech Challenge (FTC) Robotics teams. This form should be filled out by both a student and their parent. PLEASE NOTE: This is not school funded or coached.  Edina Public Schools provides meeting and work space. Parent volunteers coach the teams. Teams are formed with 6-10 students,  will need a team purchaser, a coding mentor, AND an engineering mentor.  At the moment all meetings and tournaments are planning to be in person.

The $410 Edina FTC dues + $95 Student Activity fee are due by 8/11/21.  Check the following box if you are in need of a scholarship, which is completely confidential. They are granted on a first come, first serve basis. You will receive an email confirming that you were awarded a scholarship.
