Please fill out the form below to request a hardship for the School Participation Fees for the 2020-21 School year. Hardships will be approved based on communication with the Centennial Athletic Director. All information disclosed will remain confidential to coaches and booster officials. The only information shared will be approval of hardship and percentage.

Please note that Centennial High School will work with every families' individual budgetary needs. Inability to pay will never result in inability to play or participate. High School athletics comes with financial needs and the schools need a way to fun our athletic programs, but we encourage participation for every student, regardless of personal situation.
