**This form must be completed by every parent whose student is enrolling in Pathways**

Every Pathways' student must have this form completed by a parent or legal guardian.

Our schedule includes morning courses and afternoon "studios". Although there is an in-person option, Pathways is a virtual program. As such, the state requires that we get each parent's permission for their student to take an online/virtual course in advance.

The purpose of this form is to get every parent's permission for their student to participate in school virtually/online.

Note: if you have multiple children attending Pathways, you must complete a separate form for EACH student.
