Parent/Guardian Agreement: I hereby grant permission for my child to audition for the Fairfield Choral Department and agree to the above policies and information set forth in the 2021-22 Fairfield Choral Member Handbook. I understand that the ensemble is both an academic and financial commitment. Furthermore, I pledge to positively support the Fairfield Tempo Club and music events in the district. *
Student Agreement: I understand the above procedures and requirements as well as the information in the 2021-22 Fairfield Choral Member Handbook for the Fairfield Choral Department. I hereby pledge my support and personal responsibility for being considered as a member of this ensemble and will perform to the best of my ability as an ambassador of the Fairfield City School District. *
We, the student and parent/guardian named above, understand that we must still also fill out the paper agreement that can be found on the Fairfield Choirs website (www.fairfieldchoirs.com). This agreement form must be turned in at the first rehearsal or the student will not be permitted to participate. In addition, the first payment must be paid in order to accept your membership into the program by the end of May. Audition results will be communicated to me when they are ready. *
