ASA Shooting Stars Registration Form - 2020 Summer Mini-Session

Thank you for registering for ASA's Dancers with Disabilities program, we are so thrilled to have you! Please complete this registration form in full. The credit card provided will be kept in our system and charged on payment due dates, unless we receive check/cash beforehand.

This form must be completed and filed with ASA prior to the start of classes. The form contains extremely important participant information which is necessary for ASA staff to plan and execute safe and enjoyable classes. Please answer ALL questions in their entirety.

**For dancers who may need assistance using the restroom, a parent or guardian must remain in the building for the duration of the class.

Due to COVID-19, ASA is happy to offer an in-person program or a virtual option for families who feel more comfortable staying home. Please note that ASA is taking every measure to ensure that our studio is clean and disinfected regularly. Dancers in this program will use a separate entrance into the studio to limit contact with any other patrons. We will also be holding this program during a time when there are a limited number of people inside the building. Please indicate below if you'd prefer to attend in-person or virtual. *
