**Only fill out if you have new riders or information has changed from last school year.**

- Please fill in and submit one form per student.
- Make sure you fill in all information.  
- We will contact you via email with route number and stop times.  
- If your student is shared custody and will be riding from other parents house as well,  please fill out a separate form for the secondary address.
