This form is for all CVL students who wish to return to in-person learning.

*Please know: we cannot guarantee a seat will be open in your child's home school. He or she may have to attend a different elementary school depending on current enrollment.

This form must be completed by a parent or guardian. Once it has been submitted you will receive a copy of the information and you should then wait to hear from your child's school.

** A separate form must be submitted for each child. **
