Screening and Addressing SDOH
The National Health Care for the Homeless Council will be sponsoring a learning collaborative that will occur during 4 different sessions concerning screening and the social determinates of health(SDOH). The learning collaborative aims to strengthen attendee’s knowledge on SDOH and SDOHs' impact on unhoused communities. This year we would like to open up the narrative to reflect on paramount topics such as racial equity and SDOH; workforce and SDOH; enabling services and SDOH; and we are going to highlight the importance of risk stratification. The space that is provided throughout the learning collaborative promotes conversation and open dialogue to discuss the topics at hand.

There will be formal deliberation of the barriers to screening implementation through collaborative discussion with community health partners. Throughout the sessions, a change map activity will focus primarily on the SDOH of your communities. The incorporation of collaboration and education is to contextualize the social constraints of individuals who are unhoused.

Given the disproportionate number of people who are experiencing homelessness are people of color, a synchronization to contextualize the social constraints of individuals who are non-white compared to people of color will be openly discussed. At the start of each session, a presentation on screening and addressing SDOH will be given by an expert at the forefront of care. The second half of the session will be used to address the needs of the attendees by engaging in a collaborative change map process.


The learning collaborative will comprise of at least 10 health centers with at least one staff representatives from each site. This information will be utilized to tailor four 90-minute virtual sessions. We are seeking diverse teams including RNs, Peers, Medical Assistants, Pharmacists, Social Workers, Dieticians, NPs, PAs, MDs, etc.

Come join us from December-January to learn, share, and grow together to strengthen your clinical team and improve patient outcomes. Applications are due on November 22nd.

ELIGIBILITY:  Prioritized applicants must be representatives of a Health Center Program grantee (or potential Health Center grantee)*

*Additional participants may be approved as space allows.

COMMITMENT: Accepted Learning Collaborative applicants are asked to attend a total of four 90 minute learning sessions

EXPECTED OUTCOMES:
1) Participants will be able to define at least 3 SDOH that afflicts people experiencing homelessness due to racial inequality.

2) Participants will be able to recognize at least one SDOH screening tool.

3) Participants will identify at least 3 ways enabling services helps to address social determinants of health.

4) Participants will be able to produce methods to reduce barriers while screening for SDOH through risk stratification techniques.

5) Participants will be able to identify 3 components on how pivotal workforce is address SDOH.
