Performer/Entertainer/Reader Interest Form: 4.12.24 Fundraiser
Please fill out this form to indicate interest in performing at Artful Futures: a fundraiser for The 12th House GR. We will be contacting all performers to discuss their proposed performances, and will provide contracts with the terms for participating in this event to secure and protect the interests of both parties.

Submissions are due March 15, 2024.

Please note:
-Performances should be at least 2 minutes in length, and no longer than 5 minutes in length.
-Performers will be notified of their participation by April 4, 2024, at the latest.
-Performers will not be paid by the event organizers, but are welcome and encouraged to collect tips.
-Music files and performer photos will be requested closer to the event.
-Event run time is 6-9 pm, with performances mostly happening 7-8:30 pm.
-Performers are welcome to stay for the entire event, free of charge.
