Confirmation Enrolment Form 2023
St Joan of Arc Catholic Parish has now opened enrolments for Confirmation Sacramental Program.

Enrolments will close Friday 31st March 2023 at 12 Noon.

This is a parish-based, family centred Sacramental Preparation Program that prepares children from Grade 6, who have been Baptised in the Catholic Church, and have received the Sacraments of Reconciliation and Communion.

Alongside parents as the first educators of the faith, there will be a commitment Mass and four sessions (two in the church and two online via zoom) , prior to the reception of the Sacrament. These dates are:

* Commitment Mass - Sunday 7th May (9am)

* Session dates Wednesdays 17th May - 7pm in Church; 31st May - 7pm via zoom; 14th June - 7pm via zoom; 21st June - 7pm in church

* Celebration - Sunday 25th June 11am & 2:30pm

A zoom link will be emailed on the Tuesday before the first session - if you have not received check junk/spam box as sometimes it goes there. Please note zoom link and future correspondence will be emailed to both parents.

There will be a commitment form on the website a couple of weeks before the Commitment Mass. Please print, complete and bring to the Commitment Mass on Sunday 7th May at 9am. Resources for the program will be distributed at the end of this Commitment Mass.

If your child WAS NOT Baptised at St Joan of Arc Catholic Parish and you haven't provided a copy of your child's Baptism Certificate previously to the parish please email a copy to Pastoral.Associate@stjoanofarc.org.au

If you are enrolling more than one child, please complete a Google Form for each child.

Terms and Conditions

1. I understand that I am required to attend ALL parts of the Sacramental Preparation Program.
2. I understand that my child is to attend ALL parts of the Sacramental Program and that I am required to attend with my child.
3. I understand that attempts will be made to give first preferences for ceremonies however, this will be dependent on numbers.
4. I understand that photographs cannot be take by parents or guests during the ceremony.

PRIVACY COMPLIANCE ADVICE
1. The parish collects personal information, including sensitive information about members of the faithful. The primary purpose of collecting this information is to enable it to administer the sacraments and pastoral care to the faithful, and to the children of the faithful.
Information may be required to allow the parish to meet some of its legal obligations, provide care for the child while under supervision and discharge its duty of care. The information you provide on enrolment forms may be used to request your services as a volunteer.
2. The parish from time to time discloses personal information and sensitive information to others for administrative purposes. This includes other parishes, the diocese, medical practitioners and people providing services to the schools and parish including volunteers.
3. If we do not obtain the information requested, we may not be able to administer the Sacraments.
4. Personal information collected from children is disclosed to their parents or guardians. Parents or guardians may seek access to personal information collected about them and their son/daughter by contacting the parish 9798 6657 (Office hours are 9am - 3pm Tuesday - Friday).
5. Adults may also seek access to information collected about them in the same manner.
6. As the parish is largely reliant upon locally raised funds and local efforts for its continuation, information received from those who reside in the parish or avail themselves of its services may be used to provide information about parish services, events and activities, seek donations or other forms of support from them. On occasions it uses the resources of the diocese support this process and in the course of this, information is made available to those people. We will not disclose your personal information to other third parties for other fundraising purposes without your consent.
