CCRx Donation Request Online Form
The Creative Chicago Reuse Exchange (CCRx) will accept donations of clean, safe and working surplus materials, supplies and equipment.

If you are able to bring your donation to our warehouse, that is preferred.   We are open Wednesday through Saturdays and  you can schedule a Donation Appointment at www.creativechirx.org/book-online   
Our warehouse is located in the Envision Unlimited Frick Center,  2124 W 82nd Place, Chicago, IL 60620.  

If you have a large or unusual donation or if you would like a Pick-up or Pack-up, please use this form to let us know more about it.  

We request that all donations be sorted, bundled and packed in closed, labeled boxes.  
No plastic bags, no open containers.

We will get back to you within a few business days.  One-time and/or continuing donation partnerships are welcome.
