My child can attend the following rehearsals and Performances: (Please Note, these are MANDATORY) Friday April 2, 2:15 – 3:00pm - First Preview; Friday April 30, 2:15 – 3:15pm- Dress Rehearsal; Wednesday May 5, 2:15 - 4:00pm - Dress Rehearsal; Thursday May 6, 2:15 - 4:00pm - Dress Rehearsal; Thursday May 6, 6:30pm Performance Livestream via Zoom *
ALL-CAST Rehearsals will be held over ZOOM on Mondays and Wednesdays from 2:15-3:15pm. Small group rehearsals (mostly for leads) will happen on Fridays from 2:15-2:45pm. Looking at the given schedule, list any known Conflicts. (Please note students may only have 3 absences throughout the course of the rehearsal process. Students MUST be present for Previews, Dress Rehearsals and Performances.)

Yes, I give consent for photos and videos of my child to be used and viewed by Wasatch students, teachers, admin, families and parents for the purposes related to this play.         ***Please note that due to the nature of this play’s virtual format, if YES is not marked,  the student will NOT be allowed to participate. *
I understand that there will be a $15 participation fee. If my child is cast I agree to pay this to the office by the deadline of April 1, 2021. At this time we can only accept Cash/Check payments. (Sorry, no Venmo.) *
I understand schedule updates will be made via Google Calendar and via parent Email. I agree that I can access these and check them regularly. *
I understand that this play and its rehearsals will be conducted in a virtual format, entirely over Zoom and that the students will all be participating from their own homes. I can help my child remember to attend these Zoom rehearsals and I can provide a quiet space for them to rehearse without disruptions during rehearsal times. *
I understand that rehearsal times are subject to change if/when the District moves up to Phase 3. The rehearsals and performances will still be held on the same dates, however, the TIMES will be shifted up to occur approximately 30 minutes after school ends. If we do go to Phase 3, the play will still be held in a virtual format. *
I am willing to PROVIDE A COSTME for my child who is cast in the show.  (In most cases costumes should be fairly simple and can be assembled with things the child already has, items found around the house, or inexpensive DI finds.) *
I am willing to be a Parent Helper for this show!  Please answer Yes, No, or Maybe.  If Yes, please indicate what you'd be willing to help with. (Typically in a regular show we would need parent help with building sets, organizing costumes, supervising students, etc. Since this type of virtual show is new and experimental I am not sure what kinds of help we'll need, but there will definitely still be things that pop-up that we’ll need parent help with!)

After your child's audition, I'll let you know via email if I need to see them for Callbacks!  I will also send out the CAST LIST via parent email. IMPORTANT: Please note that a child MAY still be cast, even if I don't need to see them for callbacks. So please, please, ALWAYS check the final CAST LIST!  Thank you and Good Luck!
