This form assists us as we prepare for the 2020-2021 school year, and we would encourage you to complete this form if you have a child entering first-fifth grade!  This is optional but it is your opportunity to provide any information you feel is appropriate. We ask that you remember this form is only about your child and truly helps us in making placement decisions.  

Please remember this is about your child, and so we ask that families refrain from naming specific teachers.  Due to our focus on students and creating balanced classrooms, any specific teacher request will be disregarded other than historical experience.  We encourage families to remember this is about their child so please share your expertise about your child!
