Step 2: Print, read, and have physician complete and sign Discovery Medical Form (available here: https://venture.uncc.edu/sites/venture.uncc.edu/files/2020%20Discovery%20Medical%20Form.pdf) and send to Brian Holcomb, UNC Charlotte Venture Program, 9201 University City Boulevard, Charlotte, NC 28223.

Step 3: Submit online payment for $500 to reserve your spot at https://venture.uncc.edu/discovery/online-payment, and submit the final $500 before the registration deadline of 4/10/2020
