This form is to request a private, Polaroid photo shoot with model Allie Lyke. Requests must be made at least 2 weeks in advance of potential shoot date. Please allow 1-3 business days for request approval. Unless you receive a confirmation email with an invoice attached, your request has not yet been approved.

A deposit of $150 (1/2 of total costs with the exception of travel) is required to book a private shoot. All remaining costs must be paid prior to shoot date.

No fetish, hyper-erotic, and/or pornographic requests will be approved and are strictly prohibited.
