You and your child are being asked to be in a study by Maxine Fotheringham. I am a student at the University of the Cumberlands. Colorado Springs School District 11 and your elementary school have given me permission to do my research with parents of students attending Colorado Springs School District 11. This letter is to tell you about the study so you can decide if you want to participate.

This study is to see how after school sports programs impact elementary student resiliency. It will see if after-school sports programs for elementary students increase their ability to deal with stress. You will be asked to fill out an online survey about ways your student is supported during stressful times. There will be questions about your student's age, sports played, and if the sports are at school. The survey will not include your student’s name or your name. There will be no way for the survey to be connected back to you or your student. Only group results will be shared and not individual responses. The survey results will be kept in a password-protected file and only seen by the researcher. Following the end of the study, the results of the individual survey results will be destroyed. This study is voluntary. You and your child can quit the study at any time for any reason. There are no risks to your child from being in this study. Your student's involvement in the study will not change their PE grade. Your child will not directly benefit from this study, but it may help others by creating new sports programs and programs to develop elementary kids' resiliency.
