Application deadline – June 1. An additional $25 processing fee applied if received June 1-June 15. I hereby apply for membership in the Connecticut Board of Volleyball Officials (CBVO). My application fee of $110.00 is enclosed along with a signed “Background Check Form.” I understand that I must successfully pass the written and floor examinations to be eligible for membership. I understand that the training portion of the fee ($100) is non-refundable. Membership is $100, and is due upon successful completion of the training and written & floor exams. Notification of acceptance for the CBVO training course with be on/about June 15.
