Thank you for completing this form so that we can keep track of your students schedule needs.
For the best progress, attendance at our center three times a week for the duration of your learner's scheduled programming is critical. Any interruption affects our schedule but also your child's progress.
However, we understand that family vacations and summer camps are also an important part of growing and learning and we want to do our best to accommodate this as well. Please complete the questions below indicating the dates your student will not be able to attend or would like alternate days/times (based on availability).
