Dear Parents,
 
We are very fortunate to have many programs involving our most precious gifts,  the children and youth of our community.   These programs include but are not limited to, faith formation, vacation bible school, choir and youth ministry. We would like your permission to post pictures of your child, while involved in our parish activities, on our website and on posters/bulletin boards within our community.  At no time will your child’s name be displayed with a picture and as always if you have any questions or concerns please do not hesitate to contact our staff.  This may be a repeat exercise for some of you, but we wanted to make sure to have an updated form on every child.

The policy from the Syracuse Diocese reads as follows:
Photographs and/or videos of minors are taken periodically for use in parish publications. Use of these photographs and/or videos naming those minors is strictly prohibited without the written consent of the parent or guardian.  By signing this form, you are granting permission to the parishes of Immaculate Heart of Mary and St. Joseph the Worker, and anyone acting on their behalf to use any photograph, videotape or other recording for public viewing in any print of broadcast media (e.g. radio, newspapers, magazines, brochures, websites, television, etc.) or in displays.
Please note that the Diocese, its parishes, schools and ministries have no control over the use of photographs or film taken by private individuals or the media that may be covering the event in which your child (ren) participate(s).
 
Please indicate below if your child may or may not have their picture taken and thank you for sharing our most precious gift – the youth of our parish.
