Use this form to request a registration cancellation. Please note that requests made after the stated last refund day (January 31st for Spring and July 31st for Fall) will result in a credit applied to your account that can be used for a future season. Please note any refund/credit will not include the $20 non-refundable AYSO National Membership fee and $2.75 platform service fee.
