This survey form is for incoming Clairemont High School (CHS) freshmen only; students who will be beginning 9th grade in the Fall of 2024. The information shared on this application will be carefully read and reviewed by Clairemont High School staff.  Please fill out this application form carefully and keep in mind that responses may affect your academy placement. Students are strongly urged to consult with parents/guardins before completing this. DEADLINE TO COMPLETE: FEBRUARY 2, 2024.

PLEASE NOTE: Marston 8th Graders should wait to complete this application until directed to do so by their teachers at Marston on January 24th. Choice 9th graders for Fall 2024 may complete the application until the Choice deadline closes.  Before starting this online application, please make sure you have gathered the following information, so you can complete and submit the form in one sitting. You will need the following to complete the four sections of this application:

1) STUDENT INFORMATION: student email, background, current classes, preferences for high school involvement (clubs, athletics, etc.)

2) PARENT/GUARDIAN INFORMATION: required parent/guardian contact info. including name, phone number and email

3) STUDENT PROFILE: info about student's career interests, activities interests, personality traits, and favorite school subjects.
