NOVEMBER REHEARSALS

I am aware that my child must be available BOTH Sat. & Sun. during the month of November. (This includes Thanksgiving weekend!) *
MANDATORY TECH REHEARSALS

I am aware of that if my child is cast in performances, he/she must attend ALL MANDATORY rehearsals, including studio run-throughs, theater rehearsal (Nov 28-30) and performances (Dec 1-3) or he/she will be DISMISSED.   *
ABSENCE POLICY

I understand that once my child accepts their role(s) in the production, more than one absence from rehearsal, for reasons other than sickness, may be cause for DISMISSAL from the production.               *
LIABILITY WAIVER

I am allowing my child to participating in an audition for a program offered by Pilates Dance and More Productions, Inc., herein referred to as "PDM Productions, Inc." I understand that, like any activity, participation in the performing and fine arts-related programs may present some unavoidable risk of injury, especially to people who have pre-existing conditions, illnesses, or medical disabilities. I understand that the use of various types  of  instruments, apparatuses, equipment, props, etc., also carries with it a risk of injury.  I recognize that many changes may occur as a result of  participation in any of the activities, rehearsals, workshops, or performances; including possible short-term aggravation of some symptoms, feeling of tiredness, light-headedness, increased energy, mood changes, etc. I also understand that a medical evaluation is advisable before commencing any program of physical or mental development. I have and will continue to keep PDM Productions, Inc., its owners, employees, volunteers, and contractors, fully informed of any physical or mental condition or disability which would prevent or limit my child's participation in any of the activities in which he/she am engaged. I also acknowledge that neither PDM Productions, Inc., its officers, employees, or  volunteers, are engaged in the diagnosis or treatment of any medical disease or deficiency, (whether physical, mental, or emotional).I expressly assume all risks of my and/or my child's participation in any of the programs offered by PDM Productions, Inc. and waive any claim which I might otherwise bring against Pilates Dance and More Productions, Inc. doing business as PDM Productions, Inc., its officers, directors, shareholders, employees, volunteers, sponsor organizations (including Chesterfield Dance Center, Prestige Dance Studio, and Manchester High School) or contractors as a result of any injury or illness (including COVID-19) resulting from or related to my participation in any PDM Productions, Inc. programs. PDM Productions, Inc. its officers, directors, shareholders, employees, volunteers, sponsor organizations (including Chesterfield Dance Center, Prestige Dance Studio, and Manchester High School) contractors shall not be responsible or liable for any articles lost, stolen, or damaged during, in or about any location of activities. *
PHOTOGRAPHY & INFO RELEASE FORM

PDM Productions, Inc. frequently takes photographs to help in keeping records of performances, workshops, special events, rehearsals, and various activities, as well as for use in promotions and advertisements and “photo journal”. These images may be used in print materials, PDM Productions, Inc. website and Facebook page (Nutcracker Sweet), DVDs, and other forms of social media. Everyone who participates in any of our activities may be intentionally, or unintentionally, photographed. By checking below, I hereby give PDM Productions, Inc. permission to take photographs of me and/or my child or children, for use in any and all forms of their promotions, advertisements, website, Facebook page, and any other form of social media that PDM Productions, Inc. may use. I understand that no names will accompany any of the photographs at any time without my written consent. I also understand that my/ my child or children’s image(s) will not be intentionally shared with other organizations or individuals of any kind. I also hereby give PDM Productions, Inc. permission to release my personal information which includes my contact phone number and email address to the cast of the 2021 “The Nutcracker Sweet” Production. *
COVID-19 ASSUMPTION OF RISK AND RELEASE OF LIABILITY

PDM Productions Inc. Inc is committed to providing a safe and healthy environment for its employees, dancers, and other guests. As a result, PDM Productions Inc has implemented the foregoing policies and procedures, which are subject to change based on government guidance. Although preventive measures are in place and will be adhered to, given the novel nature of COVID-19 and the rapid pace at which it spreads, PDM Productions Inc. cannot guarantee that you, your child (the “Dancer”), or your family members will not be exposed to or become infected with coronavirus. By accessing the rehearsal and performance buildings and attending the rehearsals and performances, you may be putting yourself, your child, and others at increased risk for contracting coronavirus.  
By signing below, you understand, acknowledge, and agree to the following:
 
• You assume the risk that you, your child, or your family members may be exposed to or become infected with coronavirus and that such exposure and/or infection may result in death; personal injury; illness, causing mild symptoms such as, fever or body aches, or more severe complications, such as pneumonia or organ failure; or permanent disability; and  

• You understand that the risk of exposure to or infection with coronavirus may result from the acts, omissions, or negligence of PDM Productions Inc., Prestige Dance Studio, Chesterfield Dance Studio or Manchester High School or others, including but not limited to, the faculty, staff, volunteers, guests of PDM Productions, Inc, and program participants and their families; and

• You voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to yourself, your child, or your family members (including, but not limited to, personal injury, disability or death), or any illness, damage, loss, claim, liability or expense (including medical bills, attorneys’ fees and court costs), of any kind, that you, your child, or your family members may experience or incur in connection with your child’s access to Prestige Dance Studio, Chesterfield Dance Studio or Manchester High School buildings used by PDM Productions, Inc. or his/her participation in the Program provided by PDM Productions, Inc. (collectively, “Claims”); and  

• Student and his/her assignees, heirs, parents, guardians, and legal representatives hereby release, covenant not to sue, discharge and hold harmless PDM Productions, Inc., and its officers, directors, employees, faculty, staff, students, agents, representatives, affiliates, successors, and assigns (collectively “Releasees”) of and from all Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or related to the Student’s or the Student’s family members’ or guardians’ access to Prestige Dance Studio, Chesterfield Dance Center or Manchester High School buildings or participation in the Program provided by PDM Productions, Inc., whether arising out of the negligent acts or omissions of any Releasee or otherwise, and whether any coronavirus infection or exposure occurs before, during or after access to the Prestige Dance Studio, Chesterfield Dance Studio or Manchester High School buildings or participation in the Program provided by PDM Productions, Inc,  
 
• You understand that by signing this release, you are voluntarily waiving any and all Claims, known and unknown, including those Claims that may be unknown to you, or which you do not suspect to exist at this time.  

• If any provision of this release is held to be unenforceable by a court of competent jurisdiction, then (a) this release shall be deemed to be amended to the extent necessary to render the otherwise unenforceable provision, and the rest of the release, valid and enforceable, or, alternatively (b) the invalidity or unenforceability of any provision of this release shall not affect the validity or enforceability of the remaining provisions, which shall be enforced as if the offending provision had not been included in this release. Releasees are third-party beneficiaries to this release and shall have the right to enforce it to the full extent permitted by law.

• By selecting "I understand and agree" below I acknowledge that I have read and full understand all of the terms of the COVID-19 release and waiver above, have been given the opportunity to review its terms with my legal counsel, and I am voluntarily giving up substantial legal rights, including the right to sue releasees for claims, whether known or unknown, arising out of my or my child's access to Prestige Dance Studio, Chesterfield Dance Center or Manchester High School buildings or participation in the program provided by PDM Productions, Inc

AUDITION FEE DUE TODAY

I understand that there is an audition fee of $20 per dancer that is due at time that this application is submitted. I am aware that this fee must be submitted via Venmo to @PDMProductions. Furthermore, I understand that my child's audition spot is NOT reserved until this payment has been submitted.

ONCE YOU SUBMIT THIS FORM, YOU WILL BE DIRECTED TO THE VENMO APP TO SUBMIT YOUR CHILD'S $20 AUDITION FEE. YOUR CHILD'S SPOT IS NOT RESERVED UNTIL THIS PAYMENT IS RECEIVED. PLEASE INCLUDE "AUDITION FEE" AND YOUR CHILD'S FULL NAME WHEN SUBMITTING YOUR PAYMENT VIA VENMO.
