Per the COVID-19 Updates for Meridian Athletics as of 10/05/21 Doc, student athletes who prefer to play in games without masks will be tested for COVID-19 before each game.

Parent/Guardian consent will be required prior to testing. Parental consent for testing will be granted for the entire season and is required prior to tryouts.

2. Then, please read page 1 of the Meridian School Student Consent Form for COVID-19 Testing here:

Page 2 will be completed in this Google Form.

3. Please complete this Google Form (instead of the back page of the Consent Form) before your team's first practice.
