Dear Parents and Guardians,

Please fill out the form below for each day your student is absent from Jacobsville Elementary School. Absences shall be treated as unexcused until we receive this electronic note justifying the absence in accordance with our absentee policy (i.e. vacation days are unexcused). Additional documentation is not required unless requested. Absences due to illness in excess of five consecutive days require a physician’s note. If a physician's note is required, it may be faxed the the attention of Diane Smith at 410-222-6498. You should receive a confirmation of entry upon submission. Please allow 24-48 hours processing time for this to be reflected in the AACPS Parent Portal.
