We are excited that you are planning to join us in Chicago this February. This form allows you to reserve your group's spot at the discounted rate. We realize it is hard to know exactly how many attendees you will have, so we ask that you just provide us your best estimate. You do not need to finalize your numbers and register until February 8th, 2020 (2 weeks prior to the conference).

Please note the following:

- The discounted rate per attendee is $150 for the full two-day event and $75 for a one-day pass (Regular 2-day rate is $225 and the 1-day rate is $125)
- For those booking rooms under the SEA block at the Chicago Hilton, an additional $10 discount will be provided per room night. This discount will be taken off of the finalized SEA group bill / invoice and not your hotel bill.
- To receive the discounted rate, this form must be submitted by 12/15/19
- To receive the additional hotel discount, hotel reservations must be made using the SEA code by 12/15/19. Please see SEA's website for additional information. (www.SelfEmploymentInTheArts.com). A deposit of one night's room and tax is required to book this reservation. Cancellations made within 72 hours of arrival will forfeit one night's room and tax.
- You are just providing us an estimate which helps us maximize our grant funding to its fullest. You will only be billed for the actual number of registrations that you submit in at a later date once you have them finalized.
- The full registration form(s) must be submitted by February 8th, 2020, two weeks prior to the conference. Please note that our regular cancellation policy is still applicable after this date which does allow for some additional flexibility. The full cancellation policy is available on our website and on the registration form.
