I give permission for my child to attend the Haunted Library program on Friday, October 13, 2023. I will bring my teen (7-12 grade) at 7 p.m. and my tween (4-6 grade) at 8 p.m. I understand that my child will not be allowed to exit and re-enter the program, that latecomers will not be admitted, and that I must pick up my child from the Library promptly at 9 p.m.
I understand that if my child is not behaving in a manner compliant with the Library’s Code of Conduct, I may be contacted prior to the designated pick up time. I agree that if contacted to pick up my child that I will come to the Library and take my child home.
I assert that my child is between the grades of 4 and 12. I agree not to hold the Baldwin Public Library responsible for any accidents or mishaps which may involve my child. If my child should become seriously ill or injured, I authorize you to arrange for any emergency medical care needed.
