Assessment of deaths due to COVID-19 infection
 
This survey is conducted by the Institute for Research & Development in Health and Social Care (www.ird.lk).

Up to now over 9000 COVID-19 related deaths have been reported in Sri Lanka and this has become a significant issue.

The data from the Epidemiology Unit reveals that the total number of COVID-19 infected people who have died at home so far is 1094. Also 493 COVID-19 deaths had taken place upon admission to the hospital.

The aim of this survey is to explore more information pertaining to this problem.

You can contribute to this survey if you know of any individual who passed away due to COVID-19. If you know of more than one individual, please be kind enough to enter their details separately.

Although there are no direct benefits to the families who lost their loved ones due to COVID-19, the information you provide for this survey would be extremely valuable to reduce the avoidable deaths.  

The data provided by you will be only used for this survey and your identity will be anonymized.

The results of this survey will be given to the Ministry of Health and National Operation Center for Prevention of COVID-19 outbreak, to take necessary actions for this issue.

So, the information you provide has a National importance.

It does not take more than 15-20 minutes to complete this survey.

The information you provide would be extremely helpful to enhance the current practices and procedures followed by the health care sector when controlling the prevailing COVID-19 epidemic in the country.

The survey is lead by Prof. Athula Sumathipala (Institute for Research & Development in Health and Social Care)

This survey comes under the public health surveillance category, conducted during epidemics, emergencies and disasters to gather health-related data needed for the planning, implementation, and evaluation of public health practice. Because of this reason according to the World Health Organization guidelines, ethical approval is not required for surveys of this nature.
