Student Additions to TIDE
Please use this form to make an urgent request that a student be entered into the TIDE portal for state testing.  Students must have an SSID in order to be added. In order for an SSID to be generated the following fields must be populated in PowerSchool.

  Last Name
  First Name
  Middle Name - contact EMIS if the student has no middle name on the birth certificate
  Gender
  Date of Birth
  Birthplace - This could be the city, county, or state
  Ethnicity
  Language - Primary/Home/Native language
  Admission Reason on the FS tab

You will receive an email with the student has been added.
