This form serves as the student registration for the Summer Camp Academic/Enrichment Program.
Direction to parents/guardians:
Fill out the form ONLY if you want your child to attend STEM's Summer Camp program.  Attendance is required for the full five weeks - we are willing to work with you on an individual basis.  Deadline to complete this application is May 15, 2022.  A waitlist will be available after May 15 pending open slots. Completion of this enrollment form by the parent is committing to the whole program, understanding that daily attendance will allow their child to receive full benefits. If you have any questions regarding student enrollment, please call the school office.
