即⽇起⾄2023年4⽉18⽇截⽌，報名⽅式: ⼀律使⽤表單Google Forms報名，截⽌⽇期過後Google Forms將被關閉，逾期不再受理報名。報名表單填妥交出後, 填表⼈會⾺上接到已填交表單的電郵通Notification (A copy of your responses will be emailed to the email address you provided.)。請注意: 獎狀名字將沿⽤報名表單所填名字，若獎狀名字有誤⽽需重新印製，主辦單位需加收$5⼿續費。請於收到獎⾦⽀票後三個⽉之內存帳，逾期⽀票將會失效，要求重發需加收$50⼿續費(含銀⾏⼿續費)。
**主辦單位收到⽀票後，⽅算完成報名程序。
參賽學⽣和家⻑都必須配合麻州政府防疫規定,並於賽前簽署免責⽂件。

Principal must fill out the form and submit. If the email entered dose not match with principal's email, the form will not be accepted. An email notification will be sent to principal right away. Please verify the email notification followed by sending the check to ACS. Check can be mailed any date after the notification received but not later than 4/18/2023.
