If you are not comfortable with face-to-face learning for your child you can choose the option to keep your child at home and do distance learning. If that is the option you are choosing please fill this form out so the school can prepare for your child. Please give the school a week's notice before starting distance learning. If you are ready for your child to return to the school for face-to-face instruction please fill out the form again and call the school and let them know. Once again we ask for a week's notice to prepare for your child's return. If you have any questions please call the school at 801-827-0150.
