Recommendation Letter - Info Gathering for Ms. Leick (A - Do)
INSTRUCTIONS - READ BEFORE COMPLETING SURVEY

In order for your school counselor to write you a good letter of recommendation for college admissions, we need to know more about you! Please respond to any of the following questions. It is not required that you respond to all of these questions, but remember we'll use what you share in your letter of recommendation.

Should you "Waive FERPA" in your college application? This is a common question on college applications. Waiving your FERPA rights lets colleges know that you do not intend to read your recommendations, which helps reassure colleges that the letters are candid and truthful.

Tips:
1) Avoid using ACRONYMS. What's an acronym you may ask? HHS is an acronym for Hazen High School. Please spell out in-full the names of clubs, service projects, leadership opportunities, etc.
2) Quantify things like # of community service hours performed
3) Provide specific dates for activities.

Remember: There are 450 seniors this year, and only 5 school counselors. If you want us to write you an excellent letter of recommendation, we need time to do it. Please give us at least 3 weeks to write you a letter.

What class did you most enjoy during high school? Did you have a learning experience that really motivated you, or a teacher that challenged you to do more than you ever expected you could?
Have you had a job in the past two years? Where did you work, what were your responsibilities? What did you gain from this experience (besides a paycheck :)
Describe any leadership positions you have held (elected or non-elected) and explain the role you played. What did the leadership experience mean to you?
