Parent/Guardians complete the Athletes Daily Admittance Form before EACH summer AIM event including practices, tournaments or play days.Players without a form will not be allowed to participate. NO EXCEPTIONS!

If your daughter is experiencing any of the symptoms listed in this form prior to practice, without an explanation not related to possible COVID-19, they are required to STAY HOME from practice until symptom free.
