I understand and agree to the Company Requirements, Standards and Code of Conduct

I understand and will partipate fully in the Company Class and Rehearsal Schedule

I understand that I am responsible for all Company expenses

I have reviewed the Company Performance, Activities and Event schedule and will be participating in required/mandatory rehearsals and performances.

I understand that it is my responsibility to call or email to excuse myself for missed classes and rehearsals. Failure to excuse myself from classes or rehearsals will be cause for dismissal from a show or from the company.

I understand that if I leave or am removed from the company, participation may not be considered until next season.

I understand that if I am not a company member, I will be paying full price for my tuition.

I have read and agree to the elite Liability Waiver form. The Liability Waiver form applies to all my classes, rehearsals, activities, events and performances.

Terms and Conditions of Participation

In consideration of the services of elite Dance & Theatre LLC, Paul Costales, Cheri Costales, and all other persons or entities acting in any capacity on their behalf (hereinafter collectively referred to as “elite Dance & Theatre”), I release, indemnify and discharge elite Dance & Theatre on behalf of myself, my spouse, my children, my parents, my heirs, assigns, personal representative and estate as follows.

I acknowledge that participation and performance in aerial, dance, martial arts and theatre is inherently dangerous and entails known and unanticipated risks that could result in physical or emotional injury, death, or property damage. I understand that such risks cannot be eliminated without jeopardizing the essential qualities of the activity. I accept and assume all risks existing in these activities. I grant permission to use my name and image (photo and video) in any promotional or educational publications regarding these activities.
