Transport to Treatment Survey

Cancer Council ACT has developed this survey to find out whether people affected by cancer in the ACT need assistance with transport to get to their treatment. We will use this information, as part of a larger project, to determine whether Cancer Council ACT may be able to deliver a free Transport to Treatment service in the future.

If you are undergoing treatment for cancer or a family member, friend or carer of someone who is, we invite you to please complete the survey below. All responses are anonymous.

Thank you in advance for taking the time to complete this survey and we value any additional suggestions you may like to share with us.
