Transcript Request Form
Students:  We are so happy you are moving forward in the process of continuing your pathway towards a career! In order to serve you best, please answer the following questions so we can expedite your transcript in a timely manner. Transcripts are usually sent electronically the same day they are requested. High school transcripts reflect the historical grades you have made while you are in high school, over the span of your high school career. This is what CHS school counseling sends on to the requested institution. ACT and/or SAT test scores must be sent directly from ACT and/or SAT to the institution of choice. Transcripts must be sent from the appropriate postsecondary institutions from which you earned dual enrollment credit with for any dual enrollment credits you have earned while in high school (Example:  You took Dual Enrollment US History through APSU with Mrs. Gregory. Thus you must request APSU send your transcript for the credit earned with APSU to UTK if you intend on enrolling at UTK following high school graduation).
