Hello Preschool Teachers!  We value this information from you each year as we build our classes of incoming Gurney Kindergarten students. It is very beneficial for us to have this information as it allows us to properly place your students and be prepared to meet them right where you have left off.

What follows are 18 questions about Social Skills and Self-Help Skills, 5 questions about academics and a place for your additional comments at the end of each section. Please fill out 1 questionnaire per student.

In kindergarten at Gurney Elementary, there are approximately 20-22 students of mixed abilities in a classroom with one teacher. We know sometimes a student has an atypical/significant need for support in academics and/or social/behavior areas and we purposely place him/her in a classroom with 2 adults (coteachers or teacher&aide). In your opinion... *
