Research Questionnaire on the Adverse Reaction of COVID-19 Vaccine

We, a group of Researchers kindly request you to participate in a research survey on the adverse reactions associated with COVID-19 vaccination.

The survey is for individuals that are 18 years and above ONLY. It will take less than 10 minutes to complete. The questionnaire will give you the benefit of reporting any adverse event you have experienced following COVID-19 vaccination.

Any information provided shall be stored anonymously under strict confidentiality for the purpose of research publication only. The study participation is completely voluntary. You can withdraw, choose not to answer any question or stop at any point for any reason.
