NPS COVID-19 Student Tracking During April Vacation
Starting Friday, April 16 at 3 p.m. and continuing through Monday morning April 26, please utilize the following form for reporting student positive COVID-19 cases or close contact exposure. The NPS Nurses are unavailable during the vacation week. This information will be shared with them upon the return from break.

For Testing and Quarantine purposes please take note of the following dates:
Day 0 is the last date of exposure. Your child may test on Day 5 or later. If your child tests negative and remains asymptomatic, they will be released from quarantine and may return to school and other activities on Day 8.

If you choose not to have your child tested, they will need to quarantine for 10 days and if they remain asymptomatic, they may return to school and other activities on Day 11.

Whichever option you choose, you must continue to monitor for symptoms for a full 14 days. If even mild symptoms develop or your student has a temperature of 100.0 or greater, they must immediately self-isolate and be tested.
