Form 5/Form 91 Request Form
Please use this form to request a Form 5 or Form 91 Check Ride.  This is used for all check rides - initial, renewal, and abbreviated.  

YOU NO LONGER NEED TO GET YOUR CHAIN OF COMMAND APPROVAL TO TAKE A CHECK RIDE.  

Simply complete the form below, and the Wing DOV will assign you a check pilot via email.  You should receive this assignment within a few days of completing the form.  

Form 5s are coordinated through the Wing Stan Eval Officer - Lt Col Bruce Thompson.  bthompson@pawg.cap.gov.

Form 91s are coordinated through the MP Training Officer - Lt Col Bob Gardner.  rgardner@pawg.cap.gov.
