By submitting this form I hereby grant permission for the above mentioned student to participate in the Cristina Pora Classical Dance Academy 2023/24 dance season and I certify this student is in good physical condition and is able to participate fully in this program.  I release Cristina Pora Classical Dance Academy and its teachers from all liability in case of accident or bodily injury whether it be in class, performances or exams.  I understand that all classes will be conducted in the safest possible manner by trained professional instructors. By providing my email address, I consent to email communication with the Cristina Pora Classical Dance Academy and I understand that pictures and video of my child may be used on the school website, social media or for other forms of advertising for the studio.

I understand that the Cristina Pora Classical Dance Academy can not be held liable for any exposure to the COVID-19 virus caused by any misinformation on this form or the health history provided by each Student.

REFUND POLICY
All payments are non-refundable after the due date.
No credits or refunds will be given for missed classes.
Absolutely no Partial Refunds to students who want to withdraw from a term once the term has started. Exceptions will be made only for serious illness or injury.
No transfer of fees are allowed from one student to another.
If a student’s regular class falls on a holiday or a student misses classes due to illness or
vacation, arrangements should be made to take makeup classes before the end of the term.
