In addition to filling out this NCJLA Form, sexual or physical abuse of children must be reported.

ALL PARTICIPANTS of the NCJLA must report suspicions or allegations of sexual or physical abuse
of youth athletes to local or state law enforcement & child protective services IMMEDIATELY.
