The information provided in this form is critical for grade-level planning for the 2021/22 school year.  Please complete this form for your elementary student(s)  in  TK through 6th grade.  Thank you for taking the time to complete this form.

*If you have multiple children in elementary grades TK through 6th, please complete one form for each of your children. (Example: if you have three elementary children, please fill this form out three times, one for each child.)  

*This does not need to be completed for 6th-grade students at Eagle Peak or Pomolita.

*If your child is transitioning to middle school, this form serves as the application for your middle school of choice (this means you do not need to fill out an online application in Aeries).

* There will be a lottery held for those applying to Eagle Peak.  Any Annual Enrollment Confirmation application received after February 17, 2022, will not be included in the lottery.  Families will be contacted after March 31, 2021, and if accepted, will be asked to complete an enrollment packet.
