This form should be completed if there is a long-term change in bus pick-up and/or drop-off locations. A form must be filled out for
child in the household.
NOTE: This form will not be used for a parent who needs to make a temporary change (ie. going to Grandma's today only).
When the form is submitted, it will automatically route to Ms. Tracy Cook, ECCSS Director of Transportation, with a date/time stamp.
DISCLAIMER: This form will only be processed if the student is active in Infinite Campus. Bus services may not begin for up to five days after the form is submitted. Ms. Cook will email the parent and cc the administrator in charge of buses at each school when the request has been processed.
