Bus Service Request Form
This form should be completed if there is a long-term change in bus pick-up and/or drop-off locations.  A form must be filled out for each child in the household.

NOTE:  This form will not be used for a parent who needs to make a temporary change (ie. going to Grandma's today only).

When the form is submitted, it will automatically route to Ms. Tracy Cook, ECCSS Director of Transportation, with a date/time stamp. 

DISCLAIMER: This form will only be  processed if the student is active in Infinite Campus.  Bus services may not begin for up to five days after the form is submitted.  Ms. Cook will email the parent and cc the administrator in charge of buses at each school when the request has been processed.

I understand that is may take up to 5 days for my transportation request to be processed. *
I have/will read and discussed the rules for riding the school bus with my child, realizing that bus rules for this district apply to students who are on field trips, who are being transported to athletic events, and who are being transported between home and the bus stop. *
I understand that students are allowed one pick-up location and one drop-off location. These locations may be different.  Students are not allowed to have multiple pick-up and/or drop-off locations. Students will be assigned the nearest designated bus stop to the requested address/location.  *
I understand that I am responsible for the supervision of my child(ren) at the bus stop location before the bus arrives and after students are off the bus. *
I understand student should report to the bus stop at least 10 minutes before the bus is scheduled to arrive and remain 12 feet off the road in a safe area. *
