This form should be used for obtaining records for Commission Meetings prior to March 2009 that are not available through the Public Access Portal.

All current meetings will be automatically posted online after they have been processed through our office for accuracy and ADA compliance.  Using this form to request a current meeting will NOT expedite this process.
