5th Annual Summer Bash Tournament T-Shirt Form

As a part of your registration fee, T-shirts will be provided for all players on your official team roster, the head coach, assistant coach and team manager. On the form below, please enter the number and sizes of team T-shirts that you need (equal to the number of players, coaches, on your roster).

Deadline for T-shirt orders is May  27, 2022 to get the sizes you want.

Teams that do not turn in their form by the deadline will get their team bag filled with t-shirt sizes based on sizes available at time of team check-in.
